It depends. If you implemented your modifications as template overrides, then no, the upgrade (or any update) won't effect your customized design.

In any case I'd make a backup of the file you did modify (especially the CSS ones), prior the update/upgrade.

- if we have to remove the basic version after installing advanced version?
- is that the "mailer" function has to be run on the live website? Because we are running locally

No, you don't have to remove the basic version. The advaced version will upgrade the existing installation.

If your local server is able to send emails, the mailer works locally too.
